During the webinar, a few of our dedicated workgroup members will speak about the opportunity to navigate complex discharges from the hospital, strategies to identify and help transition someone experiencing a complex discharge into their optimal discharge setting and how members of our health ecosystem, like hospitals can utilize the document to promote the health of Washingtonians.
