​This registration form is for the Gala Dinner and Awards on Saturday night. The Gala ticket will include entrance to the awards ceremony, a cash bar, dinner, live auction, live entertainment, and dancing.

NAME Refund Policy:
1. Refund requests must be submitted in writing to finance@NAMEorg.org.
3. Full refund (-$25 processing fee) is available until October 15, 2025.
4. Half refund (-$20 processing fee) is available between October 16 and November 21, 2025.
