• Gala Dinner and Awards at 6:00 p.m. on Friday night, November 21

​This registration form is for the Gala Dinner and Awards on Friday night. The Gala ticket will include entrance to the awards ceremony, participation in a raffle and live auction, live entertainment, dancing, a cash bar, and dinner.

NAME Refund Policy:
1. Refund requests must be submitted in writing to finance@NAMEorg.org.
3. Full refund (-$25 processing fee) is available until October 15, 2025.
4. Half refund (-$50 processing fee) is available between October 16 and November 18, 2025.
