THOSE IMPACTED BY THE FLOODING BEGINNING FEBRUARY 24, 2019 THROUGH JULY 3, 2019 MUST COMPLETE AND SUBMIT TO IEMA A REQUEST FOR PUBLIC ASSISTANCE FORM BY OCTOBER 18, 2019 TO BE CONSIDERED FOR REIMBURSEMENT FOR EMERGENCY WORK AND THE REPAIR OR REPLACEMENT OF DISASTER DAMAGED INFRASTRUCTURE / FACILITIES.
