This online form will capture final reports submitted pursuant to Section 5.3 and Section 5.4 of the executed GPLC Sprint Grant Agreement. 

How to Complete This Form

Final Execution Report:

• This form fulfills the Final Execution Report (Section 5.3) required under your executed GPLC Grant Agreement. Complete and submit it to GPLC within thirty days following completion of all grant-funded activities.You will receive an email confirmation of submission. Select "Final Execution Report" on the following page before continuing. 
• For the Final Execution Report, please share a final key milestone narrative, measurable outcomes, and plans related to continuing the project going forward. Separate sections exist for each of these categories. 
• Attach supporting evidence where noted (e.g. links to deliverables, photos, exhibits, etc). Photos and deliverables may be shared publicly to highlight your successes and the results of the GPLC Community Sprint Grant initiative. Please note if any documentation should not be shared.

Final Assessment Report:

• This form also fulfills the Final Assessment Report (Section 5.4), if applicable, to your grant. This report is submitted within 30 days of the one-year anniversary of the grant effective date and reports on the longer term results seen in the community as a result of your grant. 
• The Final Assessment Report is not required for all Community Sprint Grants. However, it was established to allow a bit more time for the initial project execution to take root and begin to show progress in the community. 
• If you are sharing a Final Assessment Report, please provide a narrative in the Measurable Outcomes section to highlight the longer term effect of your project in the community.  
• Please feel free to share your intentions for the project going forward at this time or to share any GPLC feedback for the operations team. This is an opportunity to come back and reflect on the process, the project, and its impact.
