Brief Survey for Presentation: Artificial Intelligence in Healthcare: Reckoning with Errors in Medical Records 

Have you ever looked as your own electronic medical record (EMR)? or the record of a family member you make medical decisions for? These days you can!  
Sometimes people find documentation errors in those EMRs. Maybe a wrong date, some missing information, or even stray information about some other patient.
 
We would like to know about your own experiences! Would you be willing to review your own medical records, as a patient (or a record you can view as someone's a legally authorized decision-maker). Tell us about the accuracy of what you see.
 
This survey is completely anonymous – no one will know who said what. Also, this is not a scientific study. We just want to know whether you see errors in your EMR. We'll share what we learn during the Northwest Patient Safety Conference on October 18, during the breakout on "Artificial Intelligence in Healthcare: Reckoning with Errors in Medical Records."
