Event Description
Using Data to Improve Health Outcomes, Reduce Costs, Drive Value, and Improve Satisfaction with Services for People with IDD
The recent focus on health equity has brought attention to the long-standing health disparities faced by people with intellectual and developmental disabilities (IDD). Left unchecked, these disparities lead to poor quality of life, preventable co-morbidities, and higher costs for state waiver programs and other organizations serving the IDD community. This presentation demonstrates how you can apply data to manage conditions known to lead to morbidity and mortality, support critical incident management systems, and drive improved member satisfaction.

Learn How To:
  • Identify claims or encounter data associated with high-risk conditions for people with IDD.
  • Utilize screening and assessment data to adapt care plans and reduce risk.
  • Flag conditions that most often lead to polypharmacy and inappropriate use of the emergency room.
Presenters:
  • Craig Escudé, MD, FAAFP, FAADM, President, IntellectAbility
  • Lorene Reagan, RN, MS, Strategic Consultant, IntellectAbility
  • Angela Peterson, CPACC, Solutions Architect, Human and Social Services, Wellsky
